A coroner has concluded that a serious failure by midwives to provide basic medical care led to the death of a baby girl.
Ida Lock sustained a severe brain injury due to a lack of oxygen during her delivery at Royal Lancaster Infirmary.
Ida’s parents campaigned for an inquest, and the Trust has faced significant criticism in the past.
Dr. Bill Kirkup, the report’s author, gave evidence at the inquest and told Sky News that it is “unforgivable” that lessons have not been learned.
